Hull taps Inhealthcare for community telehealth service

A community healthcare organisation in Hull has teamed up with Inhealthcare to deliver digital support services for people living with long-term health conditions.

City Health Care Partnership (CHCP), a community interest company (CIC) that provides community-based treatment services across Hull and the East Riding of Yorkshire, has launched a telehealth service to help people with heart failure and chronic obstructive pulmonary disease (COPD) manage their conditions at home.

The CHCP Telehealth Service offers levels of support curated to individual needs and will be underpinned by a solution provided by UK healthtech firm, Inhealthcare.

Patients are supplied with self-monitoring devices such as weighing scales, blood pressure monitors, oxygen sensors and forehead thermometers, which they can use to record health information and send the data to healthcare professionals for assessment.

Measurements are taken on a daily basis and can be sent to clinicians via a dedicated smartphone app or an automated phone call through an online portal.

Healthcare professionals can then assess readings and take appropriate action if they notice any red flags.

The telehealth service went live in March with more than 100 patients with heart failure or chronic obstructive pulmonary disease using the service.

Patients are offered an educational programme lasting up to six months aimed at coaching them on their condition and how to become more independent in managing it.

The service aims to improve the quality of life for people living with chronic conditions while minimising the need for hospital-based care, thereby easing pressure on hospital wards.

Bryn Sage, chief executive of Inhealthcare, said: ā€œBeing diagnosed with heart and lung disease is a life-changing event for people but with close monitoring patients can maintain independent and healthy lifestyles.

ā€œDigital services also offer much-needed support to the NHS by reducing the requirement for unnecessary visits to hospitals and GPs.

Harrogate-based Inhealthcare provides a number of digital health services in partnership with NHS organisations across the UK and Ireland.

The company recently announced an initiative delivered in partnership with the East Midlands Academic Health Science Network (AHSN) that aims to reduce injuries sustained by older people as a result of falls.

It also provides the technological backbone for the NHS Health Call service in North East England.
